I, the primary registrant named above, do hereby authorize adult leaders of Korean Presbyterian Church of Baltimore as agent(s) for the registered individuals above, to consent to medical care deemed advisable by an accredited physician or surgeon in an approved emergcency clinic or hospital. I further release from any liability Korean Presbyterian Church of Baltimore and any of its leaders in the event of an accident during GLF Spring Retreat, from April 4 to April 6 of 2025.
