Permission and Liability Agreement

I give permission to the child(ren) above to attend the outdoor activity with Korean Presbyterian Church of Baltimore on April 15, 2017. In case of injury to the child(ren), I will not hold legally or financially liable any leaders of the church or attending chaperones. In the case of an injury, if I am not able to be reached, I give permission for the church leaders to follow the suggested of the qualified medical professional attending to the child(ren).
