I, the parent or guardian of the named student(s) above, do hereby authorize adult leaders of Korean Presbyterian Church of Baltimore as agent(s) for me, to consent to medical care deemed advisable by an accredited physician or surgeon in an approved emergcency clinic or hospital if I am not able to be reached. I further release from any liability Korean Presbyterian Church of Baltimore and any of its leaders in the event of an accident during Youth Group Winter Retreat 2016, from December 27 to December 30 of 2016.
