TIME: Leave at church at 7:30pm, and return at 10:30pm.
(You can pick up students at the Ice Rink at 10pm)

COST: $10.00 (Ticket + Skate Rental)

I, the parent or guardian of the named student(s) above, authorize adult of Korean Presbyterian Church of Baltimore as agent(s) for me, to consent to medical care deemed advisable by an accredited physician or surgeon in an approved emergency clinic or hospital if I am not able to be reached. I further release from any liability Korean Presbyterian Church of Baltimore and any of its leaders in the event of an accident during Youth Group Bowling Night on September 28 of 2018.
